Incident:

I was moving a pallet of tinned tomatoes from the end of the warehouse to the distribution area on a fork-lift truck when the pallet became unbalanced and fell to the ground, spilling the contents. I then drove over the loose tins, resulting in a large mess on the warehouse floor.

Supervisor Comment:

Accidents will happen. Reported in loss book 14.04.18. JP

Incident:

I was packing oranges from the main crate into separate packing crates when the conveyor belt stuck. When I attempted to clear the problem and restart the belt it went at double speed, resulting in an orange pile-up at the end of the belt. Around 300-350 oranges were crushed in the incident.

Supervisor Comment:

Malfunction of belt reported to engineering. No blame attached to employee. Reported in loss book 15.04.18. JP

Incident:

I dropped a melon on the floor while unloading a delivery. The melon rolled across the workshop and got stuck between the feet of Sarah Jarvis, who was weighing a consignment of bananas. Sarah tripped, and caused two cases of bananas to fall. While chasing after the melon I slipped on the bananas and slid into the wall, hurting my wrist.

Supervisor Comment:

Small clean-up needed in weighing area. Injury to wrist not serious. Mark Watson recommended to seek medical check-up Reported in loss book 17.04.18

17.04.18 pm – No serious damage to wrist. Mark Watson to be put on light duties for a couple of days. JP

Name: Mark Watson                                      Date: 20th April 2018

Incident:

I was preparing a consignment of tinned peaches for supermarket delivery when I failed to notice the tail-lift of the delivery truck was not in the down position. I hit it with the corner of the pallet, causing the pallet truck to lurch forward and into the tail-lift. The peaches were smashed against the back of the van and there was some spillage on the floor.

Supervisor Comment:

Jesus Christ. Again!

SNAFU.

Reported in loss book 20.04.18

Recommended that Mark Watson is transferred to the front office before he destroys the building. JP
